Consumed vitamins before and after eating

Some vitamins and supplements recommended to be drunk before meals, but there is also the recommended consumed after a meal. This is because vitamins and supplements can interact with one another with drugs and even certain types of food.

But most of the vitamin might not matter consumed before or after meals.

As quoted from Nutrition.about.com and Truestarhealth, Tuesday (25/02/2010), some vitamins and supplements it can trigger toxicity when consumed together with certain types of food, and also the opposite may be harmful to the stomach if taken on an empty stomach.

Vitamins and supplements should be consumed after the meal is:

1. Hydrochloric acid supplements
Hydrochloric acid supplements work best when consumed at the beginning of a meal. Hydrochloric acid supplements are often used by people who have difficulty producing enough hydrochloric acid in the stomach when eating. When hydrochloric acid supplements consumed on an empty stomach without adding the food to be digested, then this can cause a burning sensation in the stomach.

2. Vitamin B complex
Vitamin B complex often causes mild nausea. This can be avoided by taking vitamin B complex with snacks. But do not eat greasy food when you're going to take vitamin B complex, because it will interfere with absorption.

3. Vitamins A, D, and K
Vitamins A, D, and K are fat-soluble vitamins. These vitamins require fat, either animal or vegetable to be able to optimize their absorption in the body. Vitamins are either consumed by the food and after eating.

4. Vitamin C
Vitamin C did not need the fat to dissolve in the body. But most of the vitamin C is acidic, then it should be consumed after meals to prevent irritation of the stomach, especially in patients with ulcer.

5. Zinc Supplements
Consuming too much zinc at once can cause stomach cramps and nausea. To reduce this effect, zinc supplements should be consumed after the meal.

While vitamins and supplements that can be consumed on an empty stomach or before meals are as follows:

1. Iron
Iron supplements should be consumed when the stomach to be empty alias before eating, to be absorbed by the body better. For a small dose is not a problem taking them after meals, but if the iron supplements meant for patients with anemia, it must be taken before meals and separately with other supplements, especially calcium and vitamin E.

2. Vitamin B12 and folic acid
Exclusion of the vitamin B complex are consumed together or after a meal, vitamin B12 and folic acid must be consumed on an empty stomach.

3. Proteolytic enzymes
Proteolytic enzymes like bromelain may work best on an empty stomach. Enzymes that digest proteins are often used in large quantities to help reduce inflammation. If consumed after the meal, then some of the substances of this enzyme may only end up being a digestive enzyme.

4. Bioflavonoids and phytochemicals
Many good bioflavonoids and phytochemicals are absorbed on an empty stomach. Bioflavonoids and phytochemicals have some therapeutic benefits, and consume on an empty stomach will give maximum benefit. Two examples are quercetin and vinpocetine.

5. Probiotics
Probiotics such as lactobacillus acidophilus and bifidus best consumed on an empty stomach, when you get up or before bed. This supplement is important for a healthy digestive system, and useful to treat candida, and treatment of intestinal flora after taking antibiotics.

6. Green Food
Green foods are usually best consumed on an empty stomach to take advantage of importance, namely the effect of antioxidants and nutrients.

Notes (IV) for a 2010 introduction to the Health Career Model

... Influences past, present and future

The above [development, definitions] can be explained more coherently by reflecting on events and influences at the time when Brian Hodges first created the model commuting between Manchester and his home in Sheffield in the NW England. We can compare and contrast the situation from 1983-84 and so highlight:

models of nursing
• the nursing process
• individualised care (through the nursing process)
mind mapping

grow structureModels of nursing and nursing theory are still there on nursing curricula. Students still write the essays in their first year (as I learned this week), but their importance on many nursing courses - be that foundation, the adult, paediatric, learning disability or mental health branches; or post-graduate is diminished. This does not mean to the extent that they are no longer worthy of Masters dissertations and Doctorates, but the academic agenda and theoretical emphasi*, found in the curricula, practice and management have however moved on. The nursing process is now transparent, it is embedded into theory and practice in the form of paper (the nursing Kardex) and electronic record formats. What is interesting is that individualised, or personalised care remains a challenge to achieve and measure. The final item listed above - mind mapping - is in many respects coincidental, but was concurrent and of great importance for Hodges' model, informatics and the social sciences. For decades before Hodges' model was created the potential of diagrammatic forms and representation as an educational and learning tool was recognised and championed in several quarters notably in the 1970s by Buzan and others.

So, in 2010 we can identify needs - both old and new - to account for #:
  • person-centred care
  • public engagement
  • preventive health programs (NHS = 'ill NHS')
  • public health and public mental health (in which mental health is still 'lost')
  • global health and nursing's role in achieving the aspirations of health for all (bridging other divides - local - global: glocal)
  • information technology - various schools of informatics
  • self-care
  • demographic trends
  • economics and care delivery / commissioning models
  • information and visual literacy
In addition to knowledge, in light of the above, we can also add information as a key concept to help explain Hodges' model. The website in updates after 1998 attempted to introduce information as a concept that can conjoin Hodges' model, health and social care (nursing) and informatics (in particular the socio-technical). Of the original purposes the statement concerning bridging the theory - practice gap is too general to inform this introduction. ...

* emphasi = neologism: it just sounds right?
# At the same time - is that possible?

Get fainted after eat bok choy leaves

New York City, for fans of bok choy leaves, maybe this could be a concern. Do not eat too much cabbage family vegetables because these enzymes in the vegetables can reduce the uptake of iodine into the thyroid that could make hypothyroidism (thyroid deficiency).

This condition is experienced by a female descendant of China's elderly (88 years) who live in America. The old lady was very fond eat bok choy leaves (white cabbage China), the raw numbers from 0.9 to 1.35 kg per day for several months.

These women have the belief that eating bok choy leaves can help manage symptoms of diabetes have been suffering. For months she was eating raw bok choy leaves without additional seasoning, salt or sauce.

Until one day, the woman was rushed to the emergency room at New York University's Hospital, in New York City by his family, after he became lethargic, lost the ability to walk and swallow during the three days.

Based on reports from the New England Journal of Medicine, doctors suspect the cause is an enzyme that is produced from raw vegetables.

"When I arrived at the hospital, he suffered from extreme hypothyroidism, respiratory failure associated with thyroid disorders potentially causing life-threatening coma," says Dr. Michael Chu from New York University's hospital, as quoted from AOLHealth, Monday (24/05/2010 ).

Doctors were forced to provide additional number of synthetic thyroid hormones through the blood vessels of intravenous and strong anti-inflammatory drugs for life-saving women.

Dr. Chu said these patients also have a coarse hair, dry skin and other symptoms, and given intensive treatment in ICU. Found that these patients usually consume raw bok choy leaves without additional seasoning, salt or sauce.

When the family asked if she was still able to continue to eat vegetables bok choy, the doctor did not recommend it. After four to six weeks in the hospital, she has shown progress and transferred to a rehabilitation ward.

Although bok choy vegetables can go into the stomach, but his body can not process so many leaves and stems of vegetables that contain the enzyme myrosinase. If excessive amounts of this enzyme in the body, then it could stop the production of the thyroid gland.

Someone should cook bok choy leaves before consuming it, because this condition can help make this enzyme came out and did not enter the body.

"Some kind of specific foods from the cabbage family are known to cause thyroid stop production or even increase production of thyroid hormone. This is caused by an enzyme in these vegetables may reduce the uptake of iodine into the thyroid and the iodine is needed for the production of thyroid hormones," says Dr. Chu .

Hypothyroidism occurs when the thyroid gland in the throat that controls metabolism functioning becomes less active and produce an abnormal amount of thyroid hormone or lower than it should.

The condition causes people to get tired easily, weight disorders, respiratory problems, fragile and thinning hair, dry skin and slow heartbeat. If the thyroid gland is no longer in production, it can lead to death.

Notes (III) for a 2010 introduction to the Health Career Model

Definition through purpose

The original purposes for the model's creation are as relevant today as they were in 1983-84:

1. To produce a curriculum development tool;
2. Help ensure holistic assessment and evaluation;
3. To support reflective practice;
4. To reduce the theory-practice gap.

Items 1-4 plus safety are all dependent in one way or another upon knowledge. Admittedly, this is a case of stating the obvious and something of a non-statement in that everything comes down to being knowledge (or nonsense). Resort to some global notion of 'knowledge' amounts to non-differentiation and this tells us nothing. On the contrary: this is how the simplicity of Hodges' model can cultivate and give rise to global complexity. This can help explain the model's potential and utility as a cognitive tool, an aide memoire, a mental prompt and structured conceptual checklist to frame:
  • thought
  • knowledge (ontology)
  • perspectives
  • dialogue
  • problems
  • strengths - weaknesses
  • plans and actions
  • outcomes
  • and much more ...
Whether student or specialist practitioner various conceptual elements and (care) threads can be acquired, constructed, integrated and mapped from the dual (in-situ) worlds of theory and practice to the cognitive (personal - reflective) and virtual [cogeographic?]. This means the model can be used as a mental prompt helping to inform theory as in a lecture and subsequent essay; or practically during an interview or care assessment. Beyond this cognitive application, the model's produced can then also be captured and represented on paper, or as an electronic record - by various user communities. ... (Notes IV to follow)

How to Dress your correction!

Special events can include weddings, formal events offices, social gathering, or when dating. Obviously at these events you want to show the beauty itself. The following fourrecommendations to the make-up look stunning on the show parties:

1. Super thick feather Eyes
No need much effort when the eyelashes have character. Like Miley Cyrus, direct looks sexy with a super thick lashes.

Make-up artist for Hollywood celebrities, Mai Quynh giving tips for installing false eyelashes to be perfect "Do not attach eyelashes right in front of a mirror, you should put the glass on the table and looked toward the bottom so you can more accurately in the installation of eyelashes. Also strip false eyelashes more attached to perfect "

2. Hollywood-style Retro style
Scarlet Johansson successfully implement the retro Hollywood-style appearance. Echoes the style of this beautiful celebrity. For eye makeup, use a sharp eyebrow pencil to form the eye line, drag from the corner of the eye to the outer edge with a tapered shape toward the outside. Make bold with a wash liquid eye liner. To add to the impression of classical, poleskan orange red lipstick using a brush for a more tidy and perfect.

3. Pink For Daylight
Views sweet in the afternoon with a pink made an appearance looks elegant and not excessive. See the pink hue on the cheeks a natural-looking Jessica Biel. She applies blush-on type of cream with the pink color was applied thinly by hand, this way to create a natural stick-on blush on the cheeks. Add a pink lip gloss and provide a natural tapering effect on the lashes with mascara running her transparent.

4. Smokey Eyes
Now smokey eyes "not only just black color palette. Blue, green and purple into a new color for smokey eyes and of course making the colors look more fresh.

Greenspan wins Dynamite Prize in Economics

Alan Greenspan has been judged the economist most responsible for causing the Global Financial Crisis. He and 2nd and 3rd place finishers Milton Friedman and Larry Summers have won the first – and hopefully last — Dynamite Prize in Economics.

In awarding the Prize, Edward Fullbrook, editor of the Real World Economics Review, noted that “They have been judged to be the three economists most responsible for the Global Financial Crisis. More figuratively, they are the three economists most responsible for blowing up the global economy.”

The prize was developed by the Real World Economics Review Blog in response to attempts by economists to evade responsibility for the crisis by calling it an unpredictable, “Black Swan” event. In reality, the public perception that economic theories and policies helped cause the crisis is correct.

The prize winners were determined by a poll in which over 7,500 people voted—most of whom were economists themselves from the 11,000 subscribers to the Real-World Economics Review. Each voter could vote for a maximum of three economists. In total 18,531 votes were cast.

Fullbrook cautioned that not all economics and economists were bad. “Only ‘neoclassical’ economists caused the GFC. There are other approaches to economics that are more realistic—or at least less delusional—but these have been suppressed in universities and excluded from government policy making.”

“Some of these rebels also did what neoclassical economists falsely claimed was impossible: they foresaw the Global Financial Crisis and warned the public of its approach. In their honour, I now call for nominations for the inaugural Revere Award in Economics, named in honour of Paul Revere and his famous ride. It will be awarded to the 3 economists who saw the GFC coming, and whose work is most likely to prevent another GFC in the future.”

Dynamite Prize Citations:

Alan Greenspan (5,061 votes)
As Chairman of the Federal Reserve System from 1987 to 2006, Alan Greenspan both led the over expansion of money and credit that created the bubble that burst and aggressively promoted the view that financial markets are naturally efficient and in no need of regulation.

Milton Friedman (3,349 votes)
Friedman propagated the delusion, through his misunderstanding of the scientific method, that an economy can be accurately modeled using counterfactual propositions about its nature. This, together with his simplistic model of money, encouraged the development of fantasy-based theories of economics and finance that facilitated the Global Financial Collapse.

Larry Summers (3,023 votes)
As US Secretary of the Treasury (formerly an economist at Harvard and the World Bank), Summers worked successfully for the repeal of the Glass-Steagall Act, which since the Great Crash of 1929 had kept deposit banking separate from casino banking. He also helped Greenspan and Wall Street torpedo efforts to regulate derivatives.

The poll was conducted by PollDaddy. Cookies were used to prevent repeat voting.

Hodges' model: POLITICAL domain resources

Sleep late into the night cause liver disorders

I get this health article from a popular online magazine that is about the negative impact of the habit of staying out or sleep late. I hope, this article may be useful for us.

No need to doubt, we must take reasonable precautions with no additional cost. Simply set the lifestyle and daily diet, treatment of diet and condition of time is necessary for our bodies to do the absorption and disposal of many substances that are not useful according to schedule.

Night at 9 to 11
is the time disposal of many substances is not useful or toxic dibagian antibody system (lymph nodes). During the duration of time in should be passed with a quiet atmosphere or listening to music.

If at that time a housewife is still in a condition that is not relaxed such as washing dishes or watching children learn, this can affect the health.

11 o'clock at night until 1 o'clock in the morning
during the process of dumping toxins in the liver, must take place in conditions of sleep.

Early days at 1 to 3
the disposal of toxins in the bile, also took place in conditions of sleep.

Early days at 3 to 5
dumping toxins in the paru2, because this will happen for people with a great cough cough pd this duration. Because the cleaning process (de-toxin) has reached the respiratory tract, then no need to take cough medicine so as not to impede the process of sewage.

Morning at 5 to 7
de-toxin or a little water in the large intestine, every day while in the bathroom.

Morning at 7 to 9
food nutrient absorption time for the small intestine, to eat breakfast. For a sick person should eat more in the morning ie before 6:30 o'clock. Breakfast before 7:30 o'clock is good for their health care.

For those who do not eat the morning of his wish to change this habit. Even still better late breakfast until 9 or 10 than none at all. Sleeping too late and wake up too the day will disrupt the disposal of toxins in the body useless.

In addition, from midnight until 4 o'clock in the morning is the time for bone marrow to produce blood. Therefore, en sleep tight do not be too often slept late into the night. Hopefully the tips and information about beauty and healthy useful for us all.

Health & holistic care in the 21st and 22nd Century

With the technical means at our disposal it is relatively easy to provide information to the masses. Facebook, Google and advertisers are increasingly better versed to target information not just our way, but your way.

Health education and preventive medicine programs depend on the dissemination of health and well-being information, that in turn also relies on access to an expanding digital infrastructure. Equality of access to information communications technologies is now the province of human rights and not just manifestos whether ambitious or binned.

There is much talk of trying to categorise the stage (and hence maturity) of our civilization: hence we talk about being in the information age.

In health care terms this is clearly the case. The provision of information is a key operation and goal of the UN and WHO as per the Millennium Development Goals. Amid the MDG and multiple other related initiatives we need to take care that the habits of many thousands of years do not dilute our efforts.

There is a danger that just as we have broadcast seeds to grow crops,
we will repeat the exercise with technology's reach and
so many seeds will fall on stony ground and
so not take root and
grow.

As people working in the development field know only too well, initiatives like Health Information for All by 2015 and national efforts are not just about the provision of information. In some respects that is not as big a problem as it once was (though is still presents a huge challenge) with community, urban and developmental informatics. Mobile phones and various technologies can get information to people. Increasingly though the problem is one of sufficient or extended literacy. This extends the conventional educational literacy and notion of the 3R's - Reading, wRiting and aRithmetic.

What knowledge do I need about my health and that of the 'other' gender and ethnic groups with whom I share this 'home'. What knowledge do I need in order to live long and then be able to prosper and do so sustainably?

Allied with the problem of literacy is seeking the attention of individuals. This perhaps is why infant, primary and secondary schooling are so important - learning to attend is the most important lesson of all. Especially as health and well-being messages must compete in the multi-spectral communications channels polluted with so much dross.

The MDG make clear the above requirement in Goal 2: Achieve universal primary education:

2.1. Net enrollment and retention in primary education
2.2. Proportion of pupils starting Grade 1 who also reach the last grade of primary education
2.3. Literacy rate of 15-24 year old #

With the requisite baseline - personalised literacies and the information that HIFA2015 and others can provide then individuals can achieve a level of health and well-being knowledge commensurate with living (and dying well) in the 21st century:

and 2nd-by-2nd the
growing proportion of the global population that will know the
22nd century. ...

Career and Professionalism Zone at the Health Informatics Congress 2010 (HC2010)

FROM: Sinclair, Mike (South East Coast Strategic Health Authority)

Dear all,

During the very interesting and stimulating meeting on ICT & Society run by the Faculty of Health Informatics in mid December I advertised a Careers and Professionalism Zone that we are intending to hold at this year's Health Informatics Congress (HC2010).

The aim of this zone is to increase awareness of health informatics as a career prospect for budding new students looking for career options in ICT; and to provide careers advice and guidance to those already engaged in the profession.

We are aiming to provide tables for professional organisations, and for educational establishments, as well as some one-to-one meeting facilities for individual career guidance. There will be limited space available for stands, and they will not be elaborate (to cut down on cost) as we are holding this outside the main exhibition area.

We already have further and higher education bodies coming from Sussex (who are jointly organizing the zone) and from around the Birmingham area, but would like to extend an opportunity for other educational bodies to attend (as long as it is against the purpose outlined above.)

The event is at the ICC in Birmingham and will run from Tuesday April 27th to Thursday 29th.

I attach copy of (links provided here) programme and current exhibitors booked for information.

Costs for a stand/table would be between £200 and £400 (to be firmed up at the end of this month).

If you might be interested please let me know in the first instance.
Places will be offered on a first come first served basis.

Best regards,

Mike Sinclair, Chairman

'Problem patients?' 2 - Appease me do (not)

Many of the aspirations of nursing are just that - aspirational.

Appeasement and other similar 'power' associated concepts helps explain some of the appeal of Hodges' model - with its inclusion of a POLITICAL care domain. The needs of the ONE (interpersonal care domain) are diagonally opposed by the needs of the MANY (political care domain).

Just because I may approach someone (*evidently*) abusing medication / alcohol, over-eating, risk taking ... does not mean I am prepared to continue to nurse them and hence support them in that behaviour. Attempts to engage can be made and (must be) documented, as subsequent referrals and care will build on those care encounters. There is a marked difference between those individuals above who are often socially excluded, risk takers and people who are preoccupied with their health and mental health state. (Are such people stuck in the 'sick role?) Such patients may well seek new drugs and then instantly question the medication they are taking, never satisfied, they may query their care record and care while in hospital by virtue of their personality and anxieties.

I can reject negative behaviours and attitudes, but not the person. As a member of the health care team I can explain clear terms for future engagement should the patient wish. At the end of the day we constantly review: do they have mental capacity and to what extent does their behaviour present a risk to themselves, or others...? There is also a role for specific care management to be effected, to screen and prevent people reaching emergency services when this is repeated and unnecessary. The combination of some conditions such as long term respiratory problems and anxiety can create acute management problems, both for the individuals concerned, their family carers and care providers.

People do have choices to make, and so must take responsibility for how they exercise those choices.

Crucially this also needs to be explained to referrers - e.g. general practitioners / family physicians. For effective care management the inclusion of paramedic, crisis, social and intermediate care services in care management communication and coordination is also essential.

So, there is absolutely no need for a "current model of appeasement based care".

(This is a wind-up - surely? If not I am available for career advice.)

Yours truly and the patient's (even if it hurts),

Peter Jones

Condition of Pregnant Women at the beginning of the month

Pregnant women are usually only realized she was pregnant when it will enter the second month. Although the body shape has not changed but already there are signs that different than when not pregnant. What happens during the first trimester of pregnancy?

Changes experienced by the expectant mother and fetus development is different each month. Because it recognize anything that happened during the first trimester of pregnancy, as quoted from WebMD, Thursday (04/29/2010), namely:

Month unity

Mother Condition:

1. Most women did not know she was pregnant in the first month of this.
2. But usually the body begins to feel fatigue often caused by hormonal changes and fluctuating emotions that the mother began to get angry or happy all of a sudden.
3. Morning sickness in the form of nausea or vomiting began to occur, changes in appetite and weight loss are not realized.
4. The prospective mother is also experiencing increased production of the hormone progesterone and breast began to feel soft. But still no enlargement of the abdomen.

What to do:

1. In this month should mothers begin to avoid any kind of canned or processed foods, cooked meats, tape, alcohol and soft drinks.
2. While sports can do is walking, swimming and yoga.

The development of the fetus:

1. Amniotic sac that is water tight sac around the fertilized egg cell has been formed. These conditions help to cushion the embryo develops during pregnancy.
2. In addition placenta also started developing since the first month. Spherical and functioning placenta nutrient transfer from mother to infant and also carries waste from the baby.
3. Blood cell circulation started taking shape and will start and established the existence of dark circles for eyes. At the end of the first month of pregnancy the baby was measuring around 6-7 mm (the size of rice).

Second month

Mother's condition:

1. Not too many changes that occur and are usually known to the pregnancy began through the examination of urine with a test pack.
2. Started having frequent urination due to enlargement of the uterus or womb and sometimes a bit annoying at night.
3. Felt pain in the breast and blood vessels began somewhat shaded in the breast.
4. Sometimes suffer from constipation due to increase in the hormone progesterone in the body system.

What to do:

1. Try to eat foods that are easily digested in small portions but its frequency is often
2. Expand drink some water and fruit to avoid dehydration.
3. Should stop exercise if you have shortness of breath, dizziness, soreness around the vagina or the waist and there are flecks of blood.

The development of the fetus:

1. With increase in the month, the fetus begins developing facial features.
2. Starting a little fold of skin for ears at the side of the head
3. Tiny buds appear which will develop into arms and legs and began to form the toes and hands are small.
4. Neural tube (brain, spinal cord and other nerve tissue of the central nervous system) began to form properly.
5. Digestive tract and senses begin to develop tools, bones begin to form that replaces the cartilage. Movement of the embryo begins even though the mother can not feel it.
6. The fetus has a length of about 2.54 cm and weighs about 90-10 grams, and a third of the baby's body had begun to form that consists of the head.

Third Month

Mother's condition:

1. Changes in the mother's body had begun to appear, such as the breast grows
2. Increased appetite accompanied by weight gain and stomach is a bit bigger.
3. Nausea and vomiting (morning sickness) are already starting to diminish.

What to do:

1. At this time required intake of protein, minerals and vitamins are enough to help the growth of the fetus.
2. Prospective mothers can use maternity clothes even though her stomach is not too large, this is to reduce the pressure that occurs in the stomach.
3. If mothers have difficulty sleeping (insomnia), try to eat the warm milk at night.

The development of the fetus:

1. Baby's body was almost fully formed.
2. Baby already has arms, hands, legs, mouths and fingers that can open and close fist.
3. The baby's nails have started growing and the external ear of a baby is formed.
4. Baby's reproductive organs are also growing, but the sex of the baby is still difficult to distinguish if seen in ultrasound.
5. The circulatory system and urinary system also began to work and heart can produce bile.
6. At the end of the third month, the length of the fetus is the size of about 7.6 to 10 cm and weighs about 28 grams.

'Problem patients?' 1 - Wimps and space to care

I responded to a mail list discussion around 'problem patients' which began with the following main points:
  • a relatively new nurse
  • surprise that a significant minority of (my) patients are pathological wimps;
  • even prior to seeing them they can often easily be spotted by examining their medical records;
  • for example, I frequently notice that wimps have an obscene (and often downright odd) amount of special meal requests;
  • current nursing philosophy encourages nurses to be endlessly supportive of wimps. i.e. to follow the often demonstrably wrong idea that "the patient is always right.";
  • IMO nurses who claim they benefit such patients using the current model of appeasement based care are co-dependent personality types who enjoy feeling needed more than they enjoy actually helping people.
My first response is copied below with some additional points a further post will follow:

Patients are always a challenge in that they come in lots of 'varieties'. This is why we recognise the need for individualised, person-centred care. One variety is physical trauma laden to which the full-complement of the multidisciplinary team must respond. These patients and *their* crises bring out the best in us in terms of the skills and knowledge, team work they force us to exercise.

Among the plethora of other varieties there are those who are viewed as 'problems'. Like the Pepsi ad of decades ago they are variously and pejoratively described as -

multiattendin, attentionseekin, patiencesappin, bedblockin,
buzzabuzz-buzzin, heartsinkin, timewastin,
symptomfindin, carenumbin

....
patients!

If I receive referral information or heads up information on diagnosis of a 'tci' (to come in) that suggests the above is on the way, has arrived or worse "is on your caseload" - what do I do?
  • Brace myself for impact?
  • Go off sick (suffering loss of job satisfaction)?
  • Become purely task or disease focussed, give up on effecting +ve change and improved outcome?
  • Share collective anecdotes in the office - staff changing as a way to cope, de-stress, inject some humour?
OR do I -
  • Avoid labelling them, or use these labels in a re-constructive way?
  • Ask why are they 'who' they are?
  • Gaze into their 'life history' and help them learn from it?
  • Refuse to make gross assumptions, even based on previous experience with client - patient?
  • Look at the individual wholistically - socially, educationally, behaviourally?
  • Believe you can still make a difference (be the fly-half you can be and play ball)?
  • Side step being tripped by foibles, behaviours and blatant displays of -ve obstructive ...... attitude that offend 'me'?
  • Enter their space and do my utmost to find room for manoeuvre (this is the hardest test)?
  • Speak to my manager(s) very tersely about protecting 'me' as a scarce resource and shout "OK where the hell is the gate keeper!"
  • Or, as a nurse do you boldly go and seek out new strengths and new opportunities in the same way that Kirk, McCoy, Spock et al. (2264) go and seek New Life, New Civilizations...?
You may have an impossible lock to pick (in its most severe form this has become known as 'personality disorder'), but the nursing challenge is there in all its personal and professional glory. Address this personal - under-the-skin - slant - seek supervision. Be aware of the pit that some people fall into. The trap for some people with life chances they may have completely:

missed, never had, were stolen, denied,
took for granted, spurned,
totally - wasted.

Around the pit is the zone of judgement, but beware it is a singularity (no perspective) and very slippery.
  • Otherwise go do your job: Nurse.

Morning Sickness Could Descending

Symptoms of nausea and vomiting in the morning or the usual morning sickness experienced by pregnant women under the age of 6 months. Morning sickness is a disease that turned out to be derived by gene alias descent.

Norwegian researchers reported daughters of mothers who have severe morning sickness probably three times through the same thing.

One form of morning sickness is hyperemesis gravidarum, a condition that involves nausea and vomiting before reaching 22 weeks gestational age. In severe cases can cause weight loss.

More than 2 percent of pregnant women experience this and be a common cause of pregnant women treated at the hospital. It is also associated with premature births low birth weight dams.

In a recent study led by researchers from the Norwegian Ase Vikanes Institute of Public Health in Oslo, showed that there was a strong effect of the mother's genes that descended to his daughter on the development of this condition. The results of this study have been published on 30 April at the issue of British Medical Journal (BMJ).

"This condition is influenced by the mother's lineage and followed by a factor of body mass index, smoking habits, infections and nutritional factors that can contribute to the development of hypermeresis gravidarum," said Vikanes, as quoted from HealthDay, Monday (04/03/2010) .

Previously, the condition hypermeresis gravidarum allegedly caused psychological problems, such as rejection of the subconscious to the presence of a conceived child or partner. Then the researchers tried to see whether genetic factors also influence this.

For this study, Vikanes 2.3 million and the team analyzed data from 1967 to 2006 birth years. There were about more than 500,000 pairs of mothers and children of women who experience this. If a mother has the condition, then his daughter three times more likely to develop this condition.

"These findings may add new insight into hypermeresis gravidarum and be a reference to a woman will have the possibility of experiencing the condition hypermeresis gravidarum by looking at the history of his family," he said.

NT: Nurses urged to use checklists to reduce human error in practice

Last month Nursing Times included an item on checklists, highlighting work by the WHO in perioperative nursing and the Patient Safety First campaign. Following surgery's lead is the realisation that similar checklists have the potential to improve communication and team working in other areas such as nutrition, situational awareness and pressure ulcer care.

Checklists are ubiquitous in our day-to-day lives, the dividend for using them is fairly obvious. What is less obvious is the fact that checklists alone are dead bureaucratic adornments. Checklists are animated by situations and thinking, reflective individuals in those situations. As Lomas reports there is a need to stop at an agreed key point in the patient pathway and ask the critical - checklist - questions. In team work this co-ordinated acknowledgement to complexity is essential for comprehensive, consistent and safe care.

What is interesting here is the emphasis on human factors. While checklists undoubtedly have a demonstrated - proven - role to play in safety, situational awareness is not a product of checklists alone. There is a need for learning and for the cultivation of cognitive lists. While human memory is fallibility incarnate - hence the role for checklists - there is also the need for skilled, knowledgeable professionals with the required communication and observational skills. The need for nurses to further develop observational skills has also been highlighted over recent months.

So as checklists are checked and logged: prepare a space for the mental checklist that is Hodges' model. The model is situated, person-centered and one of the original purposes was to help bridge the theory - practice gap. Try doing that with a checklist: alone

Clare Lomas Safety checklists could cut 'human error' in clinical practice, Nursing Times, 106, 3, 26 January 2010, p.2.

Toddler Behavior which is suspect

Every parent would want his son was born with flawless. But some disturbance of the body sometimes appears after the child was born. For that to know your child's behavior that must be suspected by the parents.

Not all parents consider their children's behavior. Some people consider developmental delay experienced by children is something normal and natural. Though there are certain behaviors from the child that indicates a disturbance in the body.

As quoted from Familydoctor.org, Monday (05/03/2010) there is some behavior of children who should arouse suspicion of the condition of autism, ADHD, deaf and dumb, namely:

Kidal people Creative Thinking and Faster

People are always taught to use his right hand when shaking hands, eating, writing, sports, or other work. But many artists, athletes and even world leaders who have a habit of lefty aka accustomed to using his left hand. Is it true lefty faster and more creative?

The left hand was attached to the assumption 'the devil's hand' or 'damn hands'. And since young parents have taught their children to always use the right hand to do things better.

Many parents in the 70s and 80s that forced left-handed child to use his right hand. But now note that the lefty is superior, like a fighter pilot or the ability to talk and drive at the same time.

According to Scientific American, there are about 15 percent of people actively using the left hand or left-handed. The reason some people are left-handed is not entirely clear, depending on the mix of genetic and environmental factors.

A study published in the journal Neuropsychology indicate that left-handed people process some stimuli more quickly than people who use your right hand. It also makes the left-handed people more creative.

Research conducted at the Australian National University (ANU) seem to support previous studies indicating that the habit of using right or left hand are determined from the womb.

Results showed that left-handed people more quickly and creatively because it uses both hemispheres of the brain, unlike the people who used to use his right hand which only uses the left brain.

The two hemispheres of the brain actually almost the same, and mostly for processing the same information, with data passing back and forth between them, especially through the major neural pathways.

However, certain tasks, such as language processing, tend to occur in one hemisphere only. For most people, language processing occurs on the left.

For left-handed people, these tasks may occur in both hemispheres.

Another specialty is the processing of sensory data, usually data collected on the right side of the body, such as the right eye, right ear, and others, will go to the left hemisphere for processing. And vice versa, the data collected in the left hemisphere will be heading the right hemisphere.

This study shows that left-handed people who used to write with his left hand might have a brain that is more conducive to simultaneous or stimulation and processing in both parts of the brain. Southpaw easier to use both hemispheres of the brain to manage the stimulus, so that the entire process and faster response time.

This also means that when one hemisphere of the brain gets overloaded and starts to slow, the other hemisphere could more easily choose to fill that void.

The experts also theorize that the left-handed people are mentally better when in old age and when the brain as a whole began to wane.

And as quoted from eHow, Monday (01/24/2010), left-handed people tend to be more athletic, has a better spatial awareness and think faster.

According to a study conducted by Dr. Alan Searleman from St. Lawrence University in New York, show that lefty is capable of supporting the left side of his body for all physical activities.

Southpaw also two times more skilled in problem solving and has an IQ higher than active people with the right hand.

Some famous left-handed people like President George HW Bush, Bill Clinton and Barack Obama, Prince Charles and Prince William of England. Musician Jimi Hendrix, Kurt Cobain and Paul McCartney. Scientist Isaac Newton, Marie Curie and Benjamin Franklin. Artists Michelangelo and Leonardo Da Vinci, and historical figures Alexander the Great, Charlemagne and Julius Caesar.

Drupal musings 1: To Do List - Not To Do List

I have not written about my tinkerings with Drupal for a long time. Now I've new reading glasses with occupational lenses to boot I can hopefully make some proper headway.

The new PC is going to have to wait a while, so although the PC can and does run WAMP and Drupal I'm using a refurbished 15" Macbook Pro running MAMP to explore the budding functionality of Drupal 7.

As posted under Drupal on W2tQ ages ago I have taken the old html pages:
  1. My introduction to Hodges' model (1998)
  2. Brian's original course notes (I)
  3. Brian's original course notes (II) Bridging theory and Practice
- with the aim of creating an archive within the new site. I'm re-visiting this job and have stripped the pages - naked: right down to the body and p tags. Eventually I will build some styling with CSS. I need a handle on the basic functionality to offer from day 1. The plan now is to push this (personal) envelope ever further without having a repeated scratch start. Drupal really does demand persistence. That's a problem when you can't give a project like this your ongoing attention.

A couple of additions I must organise include the constant companion that will be the Drupal notebook and a whiteboard. These will be useful over the coming weeks and months. The Drupal and PHP communities through a wide variety of sources are full of tips and code that you want to capture, filter and so possibly infuse into your project. I'm fast running out of excuses for the Drupal site no-show. On the positive side I have invested a great deal already attending the European Drupalcons in '08 and '09. Meeting a great group of Drupallers in Manchester and abroad. Reviewing my notes from Szeged + Paris and now with Copenhagen August 2010 in my sites (sorry!) - this means the project To-Do-List can get lengthy and threatens to overwhelm.

So, .... the Not-To-Do-List is even more important!

Many more Drupal musings - to follow ...

Trisomy 13 diseases and tips on how to treat

Description:
Trisomy 13, also called Patau syndrome, is a genetic disorder associated with the presence of extra material from chromosome 13.

Trisomy 13 occurs when extra DNA from chromosome 13 appears in some or all body cells.

Watch TV Frequently toddler will Troubled Schools Currently

Michigan, watch television for children is now more varied and even have a TV special for children and toddlers. But parents are reminded that parents not let children watch TV for too long because it would adversely affect his ability in school.

The harmful effects of watching TV for children is not just this one time studied. Previously also found toddlers were left to watch TV for hours will make it slow speech and motor skills are not maximized.

Researchers from the Canadian and United States report that toddlers who watch too much television will likely have lower math grades and get more intimidation from friends of his age.

The report in the Archives of Pediatrics and Adolescent Medicine, researchers found that children aged 2 years watch television more than 10 year olds. And toddlers watch television usually accompanied by food and soft drinks more.

"The results of this study support the conclusions of previous studies which stated that children who are too early to know television can disrupt or destroy his attention," said Linda Pagani of the University of Montreal, as quoted from Reuters, Tuesday (04/04/2010).

Pagani and colleagues at Bowling Green University in Kentucky and the University of Michigan say children who spend more time watching television than playing with his friends, probably will lose a valuable opportunity to learn about social skills with peers .

Researchers found that every additional hour weekly television viewing is proportional to the decrease in attention in class as much as 7 percent and a decrease in math skills for 6 percent.

Also every excess hour watching television each week, a toddler has probably 10 percent experienced intimidation, about 13 percent less active, about 5 percent weight increase and 10 percent more on eating foods and soft drinks.

Special notes:
To all men and women who are dating, you should avoid adverse dating style yourself and your future.

There are several principles that need to be held and observed in dating:

a. Man / gentleman that protect and not damage the honor of the woman he loves.

b. A true and virtuous woman was trying to maintain his honor until the first night.

c. Sex did not constitute proof of love! Sex, if done before the official marriage is just a sign of sheer lust!

Associated with courtship, there are pearls that need to be contemplated by the younger generation:

Love that is based only on worldly attraction (surface), such as: good looks / beauty, wealth, intelligence, power, lineage, and sex per se, is not true love and will not last long. Only the "heavenly love" which is true love, eternal love
(Ditto Anurogo)

Let's continue the discussion about sex myth that shook the soul of man.

105. (Advertisement) Mak EROT can zoom in and tighten the breast.

106. (Advertisement) Prof Oil lubricated, direct one minute long and 6 cm large, strong, durable, tough, can be directly proved, does not work do not get paid.

107. No need to pray before working-romance. This assumption is mistaken. Should pray first, so that the resulting child is not disturbed demon forever. To the Moslems, the following prayer:
Bismillahirrohmaanirrohiim. Cor-sy-syaithoona jannibna wa-syaithoona jannibisy razaqona MAA.

The author has not found scientific evidence from a variety of "traditional medicine" above. Therefore, before trying it, it is advisable to consult with experts in their fields.

Tuberculosis or TBC is a disease caused by bacterial infection Mikobakterium tuberculosis. These bacteria are bacillus bacteria which is very strong so it takes a long time to treat it. These bacteria often infects the lungs organs (90%) than in other parts of the human body.

Symptom
Patients who are affected that result will usually have a fever but not so high that lasts longer, is usually felt at night accompanied by night sweats. Sometimes attacks such as influenza and fever are gone arises. Other symptoms, decrease in appetite and weight loss, cough for more than three weeks (can be accompanied by blood), discomfort (malaise), and weak.

Recognize Signs Childbirth Contractions

When approaching the time of delivery, pregnant women will feel the contractions of the stomach. But what kind of shows contraction of pregnant women must get to the hospital for childbirth? The delivery process usually begins when the uterus began to contract regularly and these contractions will get stronger the longer and often.

These uterine contractions to help push the baby toward the birth canal and the cervix pulled upward, thus allowing more wide open cervix in preparation for the release of the baby's head. A few women there who have experience told to go back home or take a walk because there is something that has not happened.

Most birth process will start very slowly and gently, so some questions will arise whether it was time yet.

As quoted from BBCHealth, Thursday (05/06/2010) consists of a network of female uterine muscle fibers. At the peak of contraction, muscle fibers will reach a length of shortest and intense pain that arises.

It feels like strong period pain, or like a belt tightening in the abdomen and sometimes the pain felt in the back of the back and front. Muscle fibers were then relaxed and the contractions to fade, but will return to the muscle fibers are shorter than the previous. At that time the baby is pressed and the cervix begins to open a little wider.

When to go to the doctor after a contraction?

One of the things that must be understood is not too rushed to the hospital when the contractions had just begun. Mothers should stay at home with a quiet and relaxed and follow how the plot of the contraction that occurred. Know how often the contractions are strong and appear, as this will determine when someone should immediately go to the doctor.

During the interval of waiting, the mother can do fun things that help women feel relaxed. When the contractions started lasted for 30-60 seconds and occurs every five minutes, mothers can prepare themselves to go to the hospital.

But if a pregnancy occurs for a second or third child should go to the hospital until contractions are five minutes or so, because the birth process is usually faster than the first.

The longer the contraction will be longer and start to overlap and the mother will feel the sensation to push during that time. But mothers should not do that until the doctors had ordered her to do so.

There are some signs that indicate someone is approaching time of delivery, namely:

1. Contractions are more frequent and stronger contractions usually last more than 40 seconds and can be three times within 10 minutes of contractions. Contractions are happening even faster and faster and intense.
2. Appear pink mucus from the cervix obstruction. Labor can occur at any time after this condition, there is rapid but there is also a long time. We recommend that you consult your doctor if this happens.
3. Rupture of amniotic fluid. This marked the outbreak of the membrane that holds the amniotic sac containing the baby and amniotic fluid. This condition can occur suddenly or gradually. If this condition occurs, immediately to the hospital so the baby does not have an infection or poisoning of amniotic fluid.

The relationship between mother and child more familiar than father because oxytocin

Of New York, the philosopher Aristotle once said the mother of his son's love is bigger than dad. Recent studies show the strength of motherhood involves a more complex chemical compounds.

Behavior of the latest neuroscience research shows mother and son love involves chemical compounds that are more complex than Aristotle's maxim, which involves the hormone oxytocin.

Scientists have identified the hormone oxytocin plays an important role in the bond between people. Although the behavior of this hormone in humans has not been fully understood by many researchers.

The hormone oxytocin plays a role in the human body, especially the important role of a woman who has just become a mother. This is because oxytocin may help to induce labor and stimulate the process of lactation (breastfeeding).

'Oxytocin facilitate or assist the process of childbirth and breastfeeding, because it is believed that hormones also play a role in the formation of the bond between mother and child from the mother's own behavior, "says Jennifer Bartz, assistant professor of psychiatry at Mount Sinai Medical Center in New York, such as quoted from LiveScience, Monday (10/05/2010).

The effect of the hormone oxytocin in the formation of the bond between mother and child is the reason why the bond between mother and child more strongly than his father.

Although not many things that can be understood from the work of this hormone oxytocin, but a study has shown the role of oxytocin. This hormone can help individuals to remember the faces of people they like and distinguish it with someone who does not like.

"Probably one of the things that was facilitated by oxytocin was helpful in terms of social memory, which makes the person has a choice for certain individuals," says Bartz.

Amaurosis fugax disease and tips on how to treat

Description:
Amaurosis fugax caused by allegedly occurred clumps into plaques in the carotid artery retinal artery. These clumps cause the blood supply to the retina is lost for some time. Interruption of blood supply causes patients to lose vision.

Symptoms:
Symptoms include the sudden loss of vision in one eye. This event is usually only lasts a few seconds but can take several minutes.

Treatment:
Patients are advised to get immediate medical examination of the eye. Optometrist examination conducted to determine treatment. If the examination results show that there were more than 70% blockage of the carotid artery, the patient can be recommended for surgery to remove the blockage.

Decision to perform surgery will also be based on the severity and general health of the patient. If the patient does not require surgery, doctors usually give aspirin or blood thinners.

Some of the precautions which can be done such as lowering cholesterol, lowering high blood pressure and quitting smoking can help reduce the risk of this disease.

Topicscape Care Domain links in 3D - for the PC

Topicscape Hodges modelFollowing the post on Sunday, January 24, 2010 I have tried Topicscape and the 3D rendition of my links pages which is for the PC only. Unfortunately, for me the experience is very slow as my 7 year, 2 month old Pentium 4 2.6Ghz with its equally aged graphics card is really not up to the task. If you have a recent PC with a suitably endowed graphics card*, perhaps you will see what I sadly - for the time being - am missing.

Thanks again to Roy Grubb for recognising the organisation and effort behind the four links pages I have compiled from 1998 to date. There's a great irony in my viewing the links as a 'monster', then here they are rendered by GPU's that more commonly play host to a variety of game monsters. Recently people# have let me know of their surprise and pleasure to see that there is still a role for a human hand (and reflective mind) to weave something beyond raw Google. Of course the developments in the semantic web and RDF ... can take this even further.

*I plan to correct this in the near future with a new PC sporting a CUDA capable graphics card - also for SETI @ Home et al.

#Dear Peter

Many thanks for this, I look forward to reading the material and learning more on what seems to be an interesting model.
I’ve added a link on our site to yours – the wealth of links available on your site is phenomenal!

Many thanks and kind regards
